· Social security numbers, including the numbers of the applicants spouse and minor dependent children
· The kinds of jobs and dates worked in the 15 years before the applicant became unable to work
· The name address and phone number of a person to contact if the agency is unable to get in touch with the applicant
· The workers' compensation information if receiving, including the date of injury, claim number and proof of payment amounts
· The important relevant medical information such as names, addresses, and phone numbers of all doctors, hospitals and clinics, patient id numbers, names of medicines the applicant is taking and dates and doctors the applicant has seen
